The Phantom of the Opera:
A Union worker named Kevin Butler was working on the top of a scaffold tower which was not secured properly. Butler was thrown 25 feet to the ground and suffered a fracture of his left wrist, fractures of his upper tibia and fibula in his right leg, a wound to the head, and psychological injuries. He was awarded compensation via the court systems because his injuries affected his chances at securing more work.

Jumper:
A sci-fi thriller starring Samuel Jackson, was using a mixture of frozen sand, earth, and ice for special effects in exterior set pieces. When a wall of the material broke free during dismantling and fell down upon the crew members, set dresser David Ritchie was instantly killed.

3:10 to Yuma:
This western remake with Russell Crowe and Christian Bale involves a lot of camera work around horses. One such horse took his rider crashing into a mobile camera vehicle breaking the horses leg and injuring the rider. Stuntman Deryle Lujan suffered a punctured lung, a broken clavicle, broken ribs. The horse was killed on the set shortly afterwards.

The Crow:
This movie is rumored to have been affected by the Bruce Lee family curse with Lee's son Brandon Lee actually being killed during filming. Brandon was shot and killed when a gun containing blanks cartridges was mishandled between takes in which the gun was used. Also on set a carpenter suffered severe burns after his crane hit live power lines, a grip truck caught fire, a disgruntled sculptor crashed his car through the studio's plaster shop, and a crew member accidentally drove a screwdriver through his hand.

Charlie Wilson's War:
A special effects assistant was critically injured during the making of this Julia Roberts and Tom Hanks starring film; set to release in December of 2007. During the preparation of a scene to involve a fake stinger missile the prop missile exploded.

Blues Brothers 2000:
This Dan Aykroyd and John Goodman starring comedy wasn't funny for stuntman Bob Minor. During a car crash sequence he suffered severe injuries to his head. Oddly enough director John Landis was once again at the helm, having been involved with The Twilight Zone movie deaths earlier.

Casino Royale:
In a scene where Daniel Craig was running through the streets as James Bond, he swerved directly towards a rickshaw that was holding a cameraman. The rickshaw swerved to avoid hitting the actor which resulted in the cameraman being thrown out and breaking his arm and collarbone.
