Step #1
Open the image that you would like to create this effect on.

Create a new layer: ctrl + shift + n or Window > Layers

On the new layer, you will now make a small wound-shaped selection using the lasso tool.

Fill in the shape you have just made: Shift+F5. Use a dark shade of red color in the selection. See Image #1

Select the burn tool (O), and then choose a brush with the size 200px and hardness of 12px.

Select the smudge tool (R) with a small brush and 100% hardness.
Drag the blood as shown in Image #3

After spreading the fill color in some area, select the burn tool again, but this time select a small brush of 2px-5px.

Now burn in the position where you want the wound to be (normally, it would be in the middle of blood). Image #4

After creating the wound, select a large sized brush and set the hardness to 200px. Burn the blood and wound for two-three mouse clicks.

Now select the Texturizer: (Filter > Texture > Texturizr). In the box that pops open, select the texture Burlap.Set the 'scaling' to 200px and set 'relief' to1-3px. See Image #5

Press okay, and you are done!
The blood will look realistic if the burning and smudging done correctly.
